Our Rehoboth care Inc is a Medicaid Waiver Provider

Our Rehoboth care Inc is a Medicaid Waiver Provider under the
Department of Disabilities Services, Government of the District of
Columbia, that provides in-home support services, supported living, host
homes, non-professional BSP, professional BSP, One-Time Transitional
services (see section 1913, chapter 19npf Title 29, DCMR), Direct
support professional, Vocational rehabilitation, Respite care, Day
Habilitation specialist One to One (Direct support professional) and
Direct support services. We help and support people with intellectual
disabilities  to live independently in their homes and as
active citizens in the community.


Our resolute team of professionals at Our Rehoboth care Inc. works
with each person to ensure they receive the best of care and support
according to the needs and capabilities of each person ISP/BSP as recommended
by their PCP and other licensed support professionals which the organization conduct
to make the people achieve their goals. Our Rehoboth care Inc. abides by the

provisions of a person’s Individual Support Plan (ISP) regarding services
and support recommended by their program of support.

Companion Services

Companion Services at our agency are designed to provide non-medical assistance and supervision, supporting individuals in achieving their personal goals, desires, and needs as identified in their Individual Support Plan (ISP). The objectives may encompass personal safety, promotion of independence, community integration, and/or retirement. Whether at home or in the community, our Companion Services are aligned with Person-Centered Thinking and Discovery tools, creating an environment that supports individual growth and fulfillment.

Respite care

Our-Rehobothcareinc. renders respite care services according to guidelines aim to provide short-term or time-limited, temporary relief to primary caregivers caring for persons that is enrolled in the Waiver program so as to enable them participate in their scheduled or unscheduled time away from the person. Our respite care services are rendered in the person’s home so the services can prevent gaps in the implementation of the person’s treatment plan and ensures that they continue to receive uninterrupted services and have access to community activities as described in their ISP/Plan of Care.

Host Home

The host home program allows people withdevelopmental disabilities to live with acaregiver/family of their choice other than aparent, spouse, or guardian within the community.The Host Home living arrangement is a wonderfuloption for people looking for a more family-oriented model as it provides a more structuredway of life.

Supported Living Services

Our Supported Living Services offer a comprehensive blend of habilitation, personal care, nursing, and other residential support, all within the comfort of a home serving one to three individuals. This flexible service can be tailored to include transportation, ensuring access to Waivers, and other community services and activities for everyone living in the home. As a provider, our commitment extends beyond cat, creating an environment that fosters independence and community integration.

One-Time Transitional Home

We offer beautiful and comfortable Transitional home setting that is non-recurring set-up expenses for people who are transitioning from an institutional or another provider-operated living arrangement to our-living arrangement in a private residence where the person is directly responsible for their own expenses.

Day Habilitation

Day Habilitation services aimed at developing activities and skills acquisition to support the persons we serve to assist them integrate into their community opportunities outside of their home and assist them to accomplish goal setting in alignment with their ISP/BSP. Program enhances their integration, communication skills, independence and fostering satisfaction within the person and their families.
